Join us for a presentation on the value of Advance Care Planning and an overview of End of Life options in the state of California. Register here to receive the Zoom link for this event.

Advance Care Planning is a process that enables individuals to make plans about their future health care; it is applicable to adults at all stages of life. This presentation demystifies terms such as Living Will, Durable Power of Attorney, and Dementia Directives. The concept of value worksheets is discussed to help people consider what is most important for their lives and health care so they are better able to talk with family, friends, and medical providers about their wishes.

Sue Gess, PharmD, Advisory Board Member and Volunteer with End of Life Choices California, will describe the information and support provided by this organization in regard to all legal end of life options in California. Choices outlined will include declining medical treatments, palliative care, hospice care, and voluntarily stopping eating and drinking (VSED). California’s End of Life Option Act (2016), a law that is also referred to as Medical Aid in Dying (MAiD), will be described in more detail.

This program is presented by End of Life Choices California, a non-profit organization that provides free information, support, resources and guidance on End of Life options to anyone and/or their family seeking help navigating a serious or terminal illness or simply wanting more information on future planning.
